Proof of claim form - if your employer files bankruptcy or if you need to file a claim for money owed to you, this is the form you can use to file  your claim.  For cases in the Northern District of Georgia, you should mail an original and three copies of your completed claim form to Clerk,  U.S. Bankruptcy Court, 75 Spring St., 13th Floor, Atlanta, GA 30303.
